Family Practice Physician & Urgent Care Doctor located in American Fork, UT

Dr. David Hafen was born in Rexburg, Idaho and graduated from Brigham Young University before receiving his medical education at the University of Utah School of Medicine in Salt Lake City, Utah. He then received his specialty training at the McKay-Dee Family Medicine Residency in Ogden, Utah, after which he set up practice with the American Fork Clinic in 1997 before it was known as Premier Family Medical. He has served our communities with great dedication since then and is loved by his patients. He was pleased to welcome his brother, Mark, to our clinic in 2008. Dr. Hafen is married with children.
